To place an order with Xeno Diagnostics, please follow the instructions below:
1. All NEW customers begin by filling out and returning a credit application. (Customers paying by credit card, disregard this steps 1 and 2)
2. Return completed credit application form to Xeno Diagnostics via email, fax, or standard mail.

Indianapolis, IN 462683. For those wishing to purchase products, fill out a Purchase Order, and return to Xeno Diagnostics (You may also submit a Purchase Order using your own organization's form, so long as all necessary information is present).
For those wishing to purchase services, please fill out and return an assay requisiton form. You must contact Xeno Diagnostics BEFORE sending any samples for testing.
